Thank you for taking the time to read and review my informed consent form. It ensures that this professional relationship progresses in a transparent manner, and we can move forward in our therapeutic alliance with more integrity and security.
- My name is Vanya Malixi. I have a Master of Arts in Counselling Psychology degree from Yorkville University and I am a Registered Clinical Counsellor with the BCACC (British Columbia Association of Clinical Counsellors).
- In my private practice, I provide individual, couples, and group psychotherapy. My overriding theoretical orientations can best be described as humanistic and biopsychosocial, but in practice, I employ a wide range of theories and techniques tailored to the client’s needs. These include Eye Movement Desensitization and Reprocessing (EMDR therapy), Mindfulness-Based Cognitive Therapy (MBCT), Solution-Focused Brief Therapy (SFBT), Cognitive Behavioural Therapy (CBT), and Family Systems Therapy. I have received specific training and professional development courses in all of the therapeutic modalities I use.
- Benefits from counselling tend to include increased awareness of one’s own attachment patterns and emotional defaults, improved interpersonal relationships, reduced stress and anxiety, and an increase in one’s ability to manage intense emotions.
- Risks of engaging in psychotherapy include: strong emotions sometimes arise as part of the therapeutic process; I address this by building containment into the sessions and tracking what you are able to reasonably absorb. Some people may feel quite vulnerable and/or tired during or after a counseling session, and if this is an unfamiliar feeling, it may be initially difficult. Finally, counseling can provoke changes in how you relate to other people in your life, and although this is generally viewed as a positive shift, sometimes this can provoke resistance from others.
- I will protect your personal information and will not release details about your involvement in counseling to a third party unless you authorize me in writing to do so or unless I am mandated by a court of law by subpoena.
- As my client, you have the right to access information in your clinical records or to obtain a copy of those records, for a fee of $50.00.
- As the client, you have the right to refuse particular therapeutic modalities and to withdraw consent to counselling at any time during the counselling process.
- All information provided by you, the client, to the counsellor will be kept confidential, subject to common law, and statutory exceptions. The exceptions include the following:
- mandatory reporting of suspected child abuse
- the possible reporting of risk of serious harm to self or other
- when so ordered by a court of law
- I charge a fee of $160.00 CAD (plus GST) for each 50-minute individual counselling session and $200 CAD (plus GST) for each couple/family counselling session. My fees are payable by electronic transfer at vanyamalixi@gmail.com at the end of each session. I require 48 hours’ notice prior to cancellation. Failure to give appropriate notice will result in the full fee being charged for the session unless explicitly agreed otherwise.
